GETTING STARTED
1. CONTACT US
Pediatric Therapy Specialists understands that this may be a confusing time. We know that finding the right therapist and clinic can be frustrating and tedious. We want to make sure this process is smooth and seamless from the first time you contact our clinic. Give us a call at your preferred location, or send us an email at pedtherapyspecialists@gmail.com to inquire about services and availability. Our friendly staff will be happy to assist you in getting started with therapy services.
2. COMPLETE THE GETTING STARTED FORM:
Please complete the GETTING STARTED form below. It is through IntakeQ. Be sure to complete all required fields.
3. EMAIL INSURANCE CARD
Please email copy of the front and back of your insurance card, and the insured’s driver’s license to intakes@pedts.com.
4. SEND PHYSICIAN REFERRAL
As per our therapists’ licensure, we will also need a referral from your child’s physician in order to evaluate and treat. If your child’s physician has not already sent a copy of a referral to us, please have them fax one to your preferred location.
5. BENEFITS BREAKDOWN
Once we have received your completed form, we will verify your insurance benefits and send you a breakdown with cost estimates, visit limits, and general therapy benefits. We participate with most major private insurance companies, and offer competitive self-pay rates in the event we don’t take your insurance.
6. PLACE CREDIT CARD ON FILE
Please complete the secure IntakeQ form to place your credit card on file.
7. SCHEDULE EVALUATION
Once the Getting Started form and physician’s referral are received and reviewed, we will contact you to schedule an initial evaluation. This evaluation will provide comprehensive testing consisting of various age-appropriate activities.
8. SCHEDULE TREATMENT
Once the evaluation is complete, your evaluating therapist will determine if your child qualifies for therapy, and will provide a plan of care. We will then work with you to schedule a standing appointment time that works for you and your family.
Once scheduled, your child will have a standing appointment time. We are always accepting new patients and look forward to helping you and your family reach your therapy goals!
