1. COMPLETE THE GETTING STARTED FORM

2. EMAIL INSURANCE CARD

Please email a copy of the front and back of your insurance card, and the insured’s driver’s license to intakes@pedts.com.

3. SEND PHYSICIAN REFERRAL

Per our therapists’ licensure, we will also need a referral from your child’s physician to evaluate and treat. If your child’s physician has not already sent a referral to us, please have them fax one to your preferred location (SA 210-408-7303 and Schertz 210-233-1738).

4. BENEFITS BREAKDOWN

Once we receive your completed form, we will verify your insurance benefits and send you a breakdown of cost estimates, visit limits, and general therapy benefits. We participate with most major private insurance companies and offer competitive self-pay rates if we don’t accept your insurance.

5. PLACE CREDIT CARD ON FILE

Please complete the secure IntakeQ form to place your credit card on file.

6. SCHEDULE EVALUATION

Once we receive and review the Getting Started form and physician’s referral, we will contact you to schedule an initial evaluation. This evaluation will provide comprehensive testing consisting of various age-appropriate activities. If you have any questions about the intake process, please call or email us at intakes@pedts.com

7. SCHEDULE TREATMENT

Once the evaluation is complete, your therapist will determine whether your child qualifies for therapy and provide a plan of care. We will then work with you to schedule a standing appointment time that works for you and your family.

Once scheduled, your child will have a standing appointment time. We are always accepting new patients and look forward to helping you and your family reach your therapy goals!
